Consent to Join Form

1. I was employed by Wells Fargo Bank, N.A. in a call center between January 1, 2018 and May 31, 2021.

2. I consent to the filing of this form in the public court record in this matter, and I consent to join the above-captioned lawsuit to pursue my claim against Wells Fargo Bank, N.A. for unpaid overtime wages while working in a call center with the primary duty of handling inbound telephone calls from customers between January 1, 2018 and May 31, 2021.

3. To the extent this matter remains a collective action, I consent to be bound by the Court’s orders and decisions in this case.

4. I designate the law firms and attorneys at Christina Humphrey Law, C., GessnerLaw, PLLC, and Quintilone & Associates as my attorneys to represent me in this lawsuit or will retain an attorney of my choosing to do so.

5. I consent to having counsel for Named Plaintiff Thompson pursue this lawsuit in my name and on my behalf, and I designate Ms. Thompson and her counsel to make decisions on my behalf concerning the I understand and agree to be bound by such decisions.
